Ash Wednesday 40th Anniversary Commemoration Form

*Note:  This information is being collected by CFA (Country Fire Authority) Events Team within the Communications and Stakeholder Relations Department for the purpose of identifying interest in attending Ash Wednesday 40th Anniversary Memorial Event. Your responses will be used to assist with planning for the Ash Wednesday 40th Anniversary Memorial Service to be held 12 February 2023 and to communicate with you further about the event.

There is no legal requirement for you to provide this information, however registering your interest in attending the event will assist CFA with planning, and further communication with you. This may include requests for follow-up feedback relevant to the event. The primary project team members consisting of the Ash Wednesday 40th Anniversary Memorial Working Group will have access to this identifiable data. Event staff and volunteers will be provided with the names of attendees. Name and email address may be shared with an external event invitation provider and will be stored and used in accordance with the Privacy and Data Protection Act 2014 (VIC).

The data will be stored in accordance with CFA’s privacy and data protection responsibilities with access restricted to the primary project team members and ICT staff. This data will be destroyed in accordance with our public records obligations.
